If you’re a veteran or a family member looking to obtain military records, discharge documents (DD Form 214), or service medals, the Wisconsin Department of Veterans Affairs (WDVA) Veterans Benefits Resource Center (VBRC) can help.

Services We Provide:
VBRC assists with:
- Requesting DD Form 214 and other military service records.
- Complete the WDVA 1045
- The veteran or authorized legal representative must complete the form. If the veteran has passed the spouse or child may complete the form and submit it on behalf of the veteran.
- Submit the completed WDVA 1045 to the below email address with any of the acceptable forms of identification:
- Valid state issued Driver’s License or Photo ID
- US Passport
- State or Federal DOC Identification Card
- If assistance is needed, you may call 1-800-WIS-VETS (947-8387)
After completing the WDVA 1045 and it is determined that your records are NOT on file with the state, you may submit a records request through the federal government utilizing the link below.
