Request Military Records & Medals

The Wisconsin Department of Veterans Affairs can assist with requesting a DD214 and other military service records.

For assistance, call the Veterans Benefits Resource Center (VBRC) at 1-800-947-8387, email wisvets@dva.wisconsin.gov, or use the chat.

To request records on file with WDVA, complete the form WDVA 1045 – Request for Release and/or Discussion of Veterans Records

  • The veteran or authorized legal representative must complete the form.
    • If the veteran has passed, the spouse or adult child may complete the form but must provide proof of the veteran’s death (e.g. a copy of the veteran’s death certificate) and proof of relationship (e.g. a copy of the marriage certificate for spouses or the birth certificate for children), in addition to the WDVA 1045 and proof of identification.
    • If the veteran was unmarried, either parent of the veteran may complete the form but must provide proof of the veteran’s death (e.g. a copy of the veteran’s death certificate) and proof of relationship (e.g. a copy of the veteran’s birth certificate), in addition to the WDVA 1045 and proof of identification.

Submit the completed WDVA 1045 to Records@dva.wisconsin.gov with any of the acceptable forms of identification:

  • Valid State-Issued Driver’s License or Photo ID
  • US Passport
  • State or Federal DOC Identification Card

After completing the WDVA 1045, if it is determined that your records are NOT on file with the state, you may submit a records request through the federal government utilizing the link below.

Replacement Medals

Replacement military medals, decorations, and awards are issued by each military service department.

Requests for replacement awards and decorations may be submitted to the National Personnel Records Center (NPRC) online or using the SF-180.

  • Make sure to indicate on the SF-180 that you are seeking replacement awards and decorations.

For cases involving Air Force and Army personnel (with certain exceptions), NPRC will verify the awards to which a veteran is entitled and forward the request along with the records verification to the appropriate service department for issuance of the medals.
